Service Description

This is for a quick visit such as a sports, drivers license or school physicals ONLY. This is not for booking Wellness Exams and is NOT for follow up exams or rechecks. It is also not for Lyme visits as those take a lot longer. Use the patients name when booking. If you are booking for another person, or your child, use their information, not yours. Please also provide phone number, email address and date of birth. I cannot accept Medicare patients at this time
